Request for Amendment | Digital Chapbook & Audiobook
This is my first chapbook. The work was created in response to years of misdiagnosis, misgendering, and misrepresentation within my medical record. The chapbook was submitted as an amendment to my record in early 2021 as a form of protest art. Leveraging HIPAA, I compelled the provider to canonize the work and to print it.
Following the concept of medical documentation, the poems are dated from conception, and capture my relationship to my body, medicine, and place within community over several years. Rather than progressing chronologically though, the chapbook is broken into four thematic sections - 'Water', 'False Gods', 'Love', and 'Break-Up'.
This release is designed with accessibility in mind. Three forms are included:
(1) a reflowable and screen-reader friendly EPUB version which includes clickable links to sections and poems. This also allows for text re-sizing and changing text and background colors.
(2) a visually fixed PDF version for computer viewing,
(3) an audio-book version read by the author. This is seperated into individual tracks for easy navigation, and is in MP3 format.

Content notes: This work references many types of abuse, which are largely implied rather than detailed explicitly. Recurring content includes: death of loved ones, abandonment, prescription drugs used as prescribed, surgery, swearing. "Expertise" discusses needles, with some significant imagery. "Do Not Leave Unattended" includes a gun, and death of animals. "Zap Zap Motherfucker" includes vommitting.
